Register with the Practice

Only patients living in the following areas can register with the practice:

To register with the practice you can either pick up a registration form from the practice or download the forms below and return them to the practice when they are complete.

We require the information below to compete your registration:

• Name and address of your last doctor
• The address at which you were registered with your last doctor
• If you are from overseas, your place of birth and the date you arrived in the UK
• Your NHS number – find your NHS number online
• Your signature

To register with the practice visual identification will be needed such as a passport or driving licence and one utility bill. We aim to have your registration complete within 7 working days.

A patient questionnaire will also be provided which asks questions about your lifestyle and previous medical history. The practice will invite you for a health check with the practice nurse within 12 weeks of registering with the practice and annual health checks at specific clinics should you suffer with certain illnesses.

A registration form must be filled out each individual person. Both the GMS1 Form and the New Patient Registration Form must be filled out for us to accept your registration.

We recommend all patients sign up to one of the following online services:

• NHS App
• SystmOnline

Every patient is now allocated a named GP. This may be different from the GP you usually see. This does not change the way you book appointments. It means that one GP will have overall responsibility for your care at the surgery. The practice will make reasonable efforts to accommodate any requests regarding any preferences to be with a specific GP. Should you wish to know your named GP please contact Reception.
